We are Illinois licensed and Medicare approved. It is a division of the Health Department. The .‘PLUS’ in our name means .high-tech skill combined with tender, loving care by people who are invested in the health of the community. The registered nurses, certified home health aides and other therapists work with your doctor to provide:

skilled nursing	occupational therapy	physical therapy
medical social service	personal hygiene care	speech therapy

We work closely with Rich Township, South Suburban Senior Network Providers, as well as other local service providers. We accept Medicare, Medicaid, private insurance, or direct fee as payment. Our service area includes: Park Forest, Beecher, Mokena, Peotone, Frankfort, Chicago Heights, South Chicago Heights, Crete, Steger, University Park, Country Club Hills, East Hazel Crest, Flossmoor, Ford Heights, Glenwood, Homewood, Lynwood, Matteson, Monee, Olympia Fields, Richton Parks, Tinley Park, and areas of Thornton and Lansing.
